How to Submit Documents

You can submit documents through mail or the mobile app when the system is down.

In some cases, Washington Healthplanfinder needs additional information about your household so you can enroll in coverage.
This is called being “conditionally eligible” and it happens when the systems that check your application can’t confirm some of your information.

If you’re conditionally eligible and don’t submit documents before the deadline, you and others in your household could lose your health insurance or the amount of financial help you’re getting could change.

If you need to submit documents, you’ll receive a message that lets you know what type of document we need and the due date for you to submit it.

Accepted Documents

Use the sections below to find the most common types of accepted documents for each category. There are also forms to download when you can’t submit the recommended documents. See a full list of accepted documents.


Proof of Income or Deductions

You’ll need to provide proof of income for each household member on your application.

Some example documents that prove income include:

  • Payroll statements for the last 60 days
  • The first 2 pages your most recent tax return
  • Self-employment Profit and Loss document for the past 3 months
  • Receipts showing recent payment of reported deductions

Exception to Verification for Income
You can also use this form to explain your household income or deductions.


Proof of Lawful Presence
Some example documents that prove lawful presence include:

  • Permanent Resident Card (Form I-551)
  • Refugee Travel Document (Form I-571)
  • Valid foreign passport with I-94 stamp of admission
  • Visa with I-94 stamp of admission


Proof of Citizenship
Some example documents that prove your citizenship include:

  • U.S. Passport or U.S. Passport card
  • Enhanced Driver’s license or Enhanced State ID (must say “enhanced”)
  • Certificate of Naturalization
  • Official U.S. county birth certificate


Proof of No Other Coverage
This includes coverage through the Veteran’s Administration, TRICARE, and the Peace Corps.

Some example documents that prove you don’t have other coverage include:

  • Copy of insurance policy showing enrollment end date
  • Termination letter from insurance company
  • Declaration of Ineligibility for Other Health Insurance Coverage form (below)

You can also use this form to attest that you or a household member isn’t eligible for other coverage.


Proof of Non-Incarceration
Use this form to attest that you or a household member isn’t incarcerated in a city, county, state, or federal jail or prison.


Proof of Social Security Number
Some example documents that prove your Social Security number include:

  • Social Security card
  • IRS, military, or payroll documents showing complete SSN
  • Proof of a pending Social Security Application

Proof of Tribal Membership
Submit any official document issued by a federally recognized tribe that shows that you’re a member of that tribe, such as:

  • Tribal Membership or Enrollment Card
  • Certificate of Indian Blood (must state that you’re a member or enrolled in the tribe)

Tribal Statement of Good Faith Effort
You can also use this form to explain why you’re unable to prove your membership in a federally recognized tribe or Alaska Native Corporation.

Download our mobile app to submit your documents with the snap of a photo. Sign in to your account, go to Document Center, upload your documents and you’re done.

Submit your documents by visiting

  1. Sign in to your account
  2. Click the tab “Document Center”
  3. The documents you need to submit will be under the “Documents Needed” section
  4. Click “Upload Documents”


Fax or Mail
You can also fax or mail a copy of your documents. We can’t return original copies to you. Mailing documents will delay processing.
